1. Who We Are

This website is owned and operated by Prodest Insurance Group, LLC, a licensed insurance agency with offices at 7000 West Palmetto Park Rd, Suite 300, Boca Raton, Florida 33433. In these terms, “we”, “us” and “our” refer to Prodest Insurance Group, LLC. “You” refers to the person using the site.
We are a licensed insurance agency, not an insurance carrier. We do not underwrite policies, issue insurance contracts, pay claims, or set premiums. Those are functions of the insurance company that issues your policy. Our Licensing page lists the states in which we hold an active license.

2. Eligibility

You must be at least 18 years old and a resident of the United States to use this site. If you are using the site on behalf of another person, you confirm that you are authorised to act for them and to provide their information, including any consent to be contacted.
We are licensed in a limited number of states. If you reside in a state where we are not licensed, we cannot offer, sell, or assist you with the purchase of insurance.

3. Information on This Site Is Educational

The content on this site is general information about health insurance. It is not advice about your specific circumstances, and it is not medical, legal, tax, or financial advice.
Insurance rules, plan availability, pricing, benefits, exclusions, and eligibility change over time and vary by state, county, carrier, and household. We take care to keep the site accurate and to cite official sources where we can, but we do not warrant that the information is complete, current, or applicable to you. Where this site differs from a plan document, a carrier’s materials, or applicable law, those govern.
Nothing on this site constitutes an offer of insurance, a binding quote, or a guarantee of coverage or eligibility. Coverage is only in force when a carrier has accepted your application and issued a policy.

4. Information You Give Us

When you submit a form on this site, you confirm that the information you provide is accurate and that you are the person it describes, or are authorised to provide it on their behalf.
Submitting a form is a request for contact. It does not enrol you in any plan and does not create an agency relationship or an insurance contract.
Where a form asks for your consent to be contacted by telephone, text message, or email, that consent is given by you knowingly and may be withdrawn at any time. You can withdraw it using our Privacy Request form, by telling any agent during a call, or by the methods described in our Do Not Call Policy.
How we handle the information you give us is described in our Privacy Policy.

5. Use of the Site

13. General

If any part of these terms is found to be unenforceable, the rest remains in effect. Our failure to enforce any provision is not a waiver of it. These terms, together with our Privacy Policy and Do Not Call Policy, form the entire agreement between you and us regarding your use of this site. This website provides educational information about health insurance and is a solicitation for insurance. It is a non-government website operated by Prodest Insurance Group, a health insurance agency that presents health plans, which may include Affordable Care Act (ACA) plans, private health insurance, short-term medical insurance, or supplemental insurance based on the consumer's selection. Qualified ACA plans must meet or exceed the essential benefit requirements of the Affordable Care Act; non-ACA plans are not required to provide all of the essential benefit requirements contained in the Affordable Care Act. Pre-existing condition provisions, benefit availability, limitations and exclusions vary by plan type and state. You should review all plan details and product brochures before purchase. To qualify for ACA health insurance coverage outside of the open enrollment period, you must meet special enrollment requirements.

*Eligibility for Affordable Care Act (ACA) Advance Premium Tax Credit (APTC) and cost-sharing reductions is based on annual federal income thresholds, household size, and plan availability within the applicable service area. Premium rates, plan availability, and subsidy amounts vary by state and marketplace. Final eligibility and premium obligations are determined by the Health Insurance Marketplace under Internal Revenue Code Section 36B and applicable federal regulations.
